Alpizarin is available in the form of tablets of 0.1 g. The drug is well absorbed, exhibits antiviral activity due to its inhibitory effect on the reproduction of HSV, has a high immunotherapeutic index, and stimulates cellular immunity. The use of the drug from the first days of the disease gives a positive therapeutic effect.

Alpizarin tablets are taken orally, regardless of the meal. Adults are prescribed 0.1 g, the course of treatment is 10-15 days; children - 1/2 tab. 3 times a day for 7 days. It is recommended to use in combination with local treatment.

Zovirax (acyclovir) has a pronounced antiherpetic activity. Effective in patients with herpes with immune deficiency of various origins. Zovirax for internal administration is available in vials containing 250 mg of sterile acyclovir. The solution must be prepared immediately before use by adding 10 ml of isotonic sodium chloride solution. The dose for children is calculated on the body surface area in accordance with the instructions for use of the drug.

Interferon has an inhibitory effect on a wide range of viruses. Mechanism antiviral action It consists in its penetration into the cell, followed by the synthesis of an antiviral protein, which prevents or suppresses the reproduction of viruses.

It is used as an anti-relapse agent. It has no contraindications, is available in ampoules of 500 IU / ml. Method of application: intramuscularly, 1 ml every 3-4 days, course - 4-5 injections.

Reaferon - domestic drug used for the treatment of various viral diseases: hepatitis B, influenza and its complications, ophthalmic herpes and genital herpes. Treatment with reaferon in cases of virus infection in children is carried out according to the following scheme: intramuscular injection 500,000 IU 1 time per day. Course - 5 injections with an interval of 1-2 days. After the 1st course of therapy in case of repeated recurrence and the absence of a tendency to improve the condition within 5-6 months. a second course is prescribed according to the same scheme.

After treatment with reaferon, most children have a positive therapeutic effect. Children tolerate the drug well side effects and allergic reactions are not observed. With a continuously relapsing form of herpetic stomatitis in children for 1-2 months. there is a gradual decrease in the severity of symptoms of a general and local nature, up to complete disappearance. Reaferon helps to interrupt the permanent course of the disease, increase remission, in some cases - to clinical recovery.

Decaris (levamisole) selectively stimulates the regulation of the function of T-lymphocytes, plays the role of an immunomodulator, enhancing the response of cellular immunity. In the treatment of children with recurrent herpetic stomatitis, it helps to prevent relapses. Assign children 50 mg after meals once on the recommendation of a pediatrician. Side effects with a single application are not observed. Taken in the form of tablets, 1/2 tab. per day for 3 days with a break of 5 days between cycles. In total, the course of treatment is 2-3 cycles.

The invention relates to the pharmaceutical industry, namely to an agent for the treatment of lesions of the oral mucosa with red lichen planus. Adhesive ointment for the treatment of lesions of the oral mucosa with lichen planus, containing sea buckthorn oil and 0.5% prednisolone ointment, solcoseryl dental adhesive paste and vitamin A, at a certain ratio of components. The above ointment is effective for the treatment of lesions of the oral mucosa with lichen planus. 3 Ave.

The invention relates to the field of clinical medicine, namely to dermatology and dentistry, and can be used to treat the oral mucosa in patients with lichen planus.

Red lichen planus(ICD-10 code: L43) is a chronic pruritic dermatosis of a multifactorial nature, characterized by monoform papular rashes on the skin and mucous membranes.

The lesion of the mucous membrane of the mouth and lips is characterized by the appearance of small nodules, the size of a pinhead, flat polygonal, dense, grayish-white in color with a sheen. The mucosa is most commonly affected inner surface cheeks and tongue. Eating and speaking become difficult specific smell from mouth.

Local treatment for lesions of the oral mucosa (OM) is the use of ointments using corticosteroids. In addition, in the clinic, in the treatment of lesions of the oral mucosa with lichen planus, rinses with antiseptics and local anesthetics, applications with keratoplastic agents: sea buckthorn oil, rosehip oil.

A known method for the treatment of lichen planus (LP) of the oral mucosa (patent invention RU 2356551 C1 dated March 17, 2008). To do this, Mexidol is used in tablets and in the form of Mexidol dent fito toothpaste. Mexidol tablets are administered one twice a day for a month. toothpaste use twice a day for 1-1.5 months. EFFECT: invention makes it possible to increase the effectiveness of treatment and lengthen the period of remission due to the general and local influence of mexidol on the pathogenesis of the disease.

Also known is a method of treating lichen planus by complex therapy with the use of desensitizing agents, local application to the oral mucosa of a hormonal and keratoplastic agent of 4% water-insoluble colloidal solution of chitosan with a degree of deacetylation of 0.95 molecular weight 100-120 kD, which is applied to the pre-treated with a 1% hydrogen peroxide solution and dried oral mucosa in the affected areas with a thin layer 3 times a day after meals 1 time at night for 30 days, after which for prophylaxis for another 30 days continue treatment with a solution of chitosan 1 time per day after meals and 1 time at night (patent for invention RU 2258516 C1).

A known method for the treatment of lichen planus of the oral mucosa using vitamins A, E, C, multivitamins as for general treatment, and for local (applications of oil solutions of vitamins A, E, carotenoline, rosehip oil, sea buckthorn, etc.) in order to normalize metabolic processes in the body and the epithelium of the oral mucosa.

However, this therapy is applicable only in the typical form of lichen planus, it does not take into account the peculiarities of the healing process of the affected oral mucosa in exudative-hyperemic and erosive-ulcerative forms of LP. In addition, there is a known method local treatment KPL SOPR with the use of corticosteroid ointments (sinalar, flucinar - "drugs that regulate metabolic processes, refer to the hormones of the adrenal cortex and to their synthetic analogues”), stimulating drugs (methyluracil - “drugs that stimulate metabolic processes”), and carotolin (“vitamins and related drugs, a drug of the carotene group”) was used as a keratoplastic drug.

This method of treatment has a pronounced anti-inflammatory effect and stimulates reparative processes in the oral mucosa.

However long-term use hormonal ointments can cause side effect in the form of atrophy of the epithelium, inhibition of regeneration processes, there are contraindications for use in viral infections. Stimulant and epithelizing preparations have weak adhesion to the oral mucosa and do not suppress the associated secondary infection.

Some authors note good wound healing effect when using vitamin A for the treatment of lesions of the oral mucosa in LP, the regulation of the processes of proliferation and differentiation of epithelial cells, but weak adhesion can be considered a disadvantage of the method oil solution vitamin A to ORM, lack of antibacterial action, which leads to an increase in the duration of treatment and can lead to possible relapses diseases.

The closest in technical essence and selected as a prototype is an ointment consisting of 2 components: sea buckthorn oil and 0.5% prednisolone ointment. However, the ointment applied to the mucous membrane is also easily washed off with saliva and food, so its anti-inflammatory and keratoplastic effect does not have a stable therapeutic effect, leading to accelerated healing. erosive lesions mucous membrane.

The task was to create an ointment for the treatment of lesions of the oral mucosa with lichen planus, which has a high adhesive, keratoplastic and anti-inflammatory effect.

EFFECT: increased efficiency of treatment of lesions of the oral mucosa in LP by reducing the time of epithelialization of the affected mucosa when using the claimed ointment, which combines high adhesive, keratoplastic and anti-inflammatory activity of its components.

The task is achieved by using an ointment containing sea buckthorn oil and 0.5% prednisolone ointment, solcoseryl dental adhesive paste, vitamin A, with the following % ratio of components:

The inventive ointment for the treatment of lesions of the oral mucosa in LP is prepared as follows. All the necessary components are prepared in the prescribed amount and mixed at room temperature.

For 100 g of the finished product it is necessary: to 20 g of sea buckthorn oil add

20 g of 0.5% prednisolone ointment, mix thoroughly. 40 g of solcoseryl dental adhesive paste and 20 g of vitamin A are added to the resulting composition. Mix thoroughly until a homogeneous mass is formed.

Characteristics of the components of the claimed ointment.

Sea buckthorn oil provides a keratoplastic effect due to the content of essential macro- and microelements, including vitamins E, C, B1, B2, B6, F, P, folic acid, flavonoids (rutin), tannins, normalizes local metabolism.

0.5% prednisolone ointment has anti-inflammatory, anti-allergic and anti-exudative effects.

Solcoseryl dental adhesive paste (Reg. No. 015194/01-2003) provides accelerated healing, pain relief and protection of the wound surface. In addition, the paste provides high adhesion (sticking) of the medicinal components introduced into the composition to the wet mucous membrane, and, as a result, guarantees a long-term deposition of all medicinal components of the ointment in the mucous membrane, which is necessary to stimulate the processes of regeneration, anesthesia, sorption and healing.

Vitamin A (retinol) - fat-soluble vitamin, antioxidant, participates in redox processes, regulation of protein synthesis, promotes normal metabolism, functions of cell and subcellular membranes, plays important role in the formation of bones and teeth, as well as body fat; needed for new cell growth.

The parameters of the components introduced into the composition of the proposed agent were determined empirically. The introduction of components below the lower or above the upper limits does not provide optimal impact, and also worsens the presentation.

The shelf life of the resulting ointment is 1 month. The parameters of the ointment are stable during storage. The ointment is a light yellow mass with a pleasant smell.

The technical result of the proposed solution is achieved by a combination of high adhesive, keratoplastic, anti-inflammatory and sorption activity of the components.

Therapeutic action of the proposed agent has been tested in the clinic. Complications, allergic reactions and side effects when using this ointment were not identified. The ointment is well fixed on the mucosa, even in the presence of saliva and exudate, providing long-term deposition medicinal substances on the surface of mucosal lesions, reduces the time of epithelialization of the oral mucosa.

The claimed ointment has passed clinical trials on the basis of the State Budgetary Institution "Regional Clinical venereal dispensary» Stavropol.

Examples of specific implementation.

Example 1. Patient A., 65 years old, amb. card number 1719.

Complaints of pain when eating. Objectively: on the buccal mucosa there are multiple papular elements, grayish-white in color, merging, form a mesh pattern, the mucosa is hyperimposed, painful when eating.

Diagnosis: widespread lichen planus, typical form, exudative-hypermic on the oral mucosa, was made on the basis of complaints, anamnesis, objective biopsy data. The patient received a traditional course of therapy with oral administration of stimulants (sodium nucleinate) and desensitizing agents (tavegil) and local application of hormonal drugs (sinalar) to the oral mucosa.

Treatment. For the treatment of lesions of the oral mucosa, the patient was prescribed the claimed adhesive ointment - applied 2-3 times a day to the damaged areas of the mucous membrane.

Immediately after the start of the application of the ointment, its high adhesive and keratoplastic effect was noted. After a single application, the ointment is fixed on the mucosa even after eating. The epithelialization of the affected mucosal mucosa has accelerated significantly, food intake and speech are facilitated, the patient indicates an improvement in the general condition.

On examination on the 3rd day, there is a decrease in inflammatory foci, pain persists when taking hot food, on the 5th day the mucous membrane is painless, pale pink, moist, on the 14th day there are single papular rashes, on the 21st day the papules are preserved only in the retromolar region. A month after the end of treatment, the patient does not complain, the mucous membrane of the oral cavity is pale pink, moist, along the line of closing of the teeth there are single papules without signs of inflammation, flattened.

Example 2. Patient S., 45 years old, amb. card number 1975.

The patient was admitted with a diagnosis of widespread lichen planus, a typical form on the oral mucosa. The diagnosis was made on the basis of complaints, anamnesis, objective biopsy data and contact microscopy. The patient received a traditional course of general therapy with oral stimulant (sodium nucleinate) and desensitizing (tavegil) agents.

Against the background of general therapy, the patient was prescribed the inventive ointment for the treatment of lesions of the oral mucosa - apply 2-3 times a day to the damaged areas of the oral mucosa.

Epithelialization of erosions accelerated significantly, especially pronounced was healing effect in the retromolar region, where the deepest foci were located, eating and speech were significantly facilitated, already 3 days after the start of treatment, the patient indicated an improvement in her general condition.

On examination on the 5th day, the presence of inflammatory foci was noted, the pain persists when taking hot food, on the 7th day the mucous membrane is painful, hyperimposed in the area of papular elements, on the 14th day papular rashes persist, on the 21st day inflammation decreases, papules are preserved. A month after the end of treatment, the patient complains of discomfort in the oral cavity, the mucous membrane of the oral cavity is pale pink, moist, along the line of closing of the teeth, there are single papules without signs of inflammation.

Example 3. Patient S., 45 years old, amb. card No. 12247 (according to the "prototype" method).

The patient was admitted with a diagnosis of lichen planus, a typical form on the oral mucosa. The diagnosis was made on the basis of the patient's complaints, collected medical history, objective biopsy data and contact microscopy. The patient received a traditional course of general therapy with oral stimulants (sodium nucleinate) and desensitizing agents (tavegil).

Against the background of general therapy, the patient was prescribed a prototype ointment for the treatment of lesions of the oral mucosa.

The effect of the local treatment was significantly lower than when using the developed ointment. The main disadvantage of the prototype ointment was that the applied ointment was easily washed off with saliva and food, so its anti-inflammatory and keratoplastic effect did not have a stable therapeutic effect, and the healing of mucosal lesions did not accelerate. Ointment prototype did not provide keratoplastic effect, the patient constantly complained of pain when eating and talking.

After 35 days of treatment against the background of complete disappearance clinical manifestations Papules continued to persist on the skin and in the oral cavity, which led to an increase in the duration of treatment up to 60 days.

Clinical trials of the proposed ointment were carried out in 46 patients in the period from 2009 to 2011. on the basis of GBUZ "Regional clinical venereal dispensary" Stavropol. None of the patients who used the claimed ointment for the treatment of lesions of the oral mucosa in LP, inflammatory complications was not noted, and the terms of epithelialization of erosions, on average, decreased by 50%. The high keratoplastic and anti-inflammatory effect of the developed ointment was especially pronounced due to its high adhesion to the oral mucosa.

The obtained data showed high adhesive, keratoplastic and anti-inflammatory properties of the developed ointment, allowing to accelerate the healing of the oral mucosa, prevent the development of inflammatory complications and reduce the period of complete epithelialization of the affected mucosa in LP, which allows us to recommend more wide application this ointment in the clinic.

Thus, the proposed technical solution allows to obtain an inexpensive, cost-effective highly effective remedy for the treatment of lesions of the oral mucosa in lichen planus, as well as to reduce the treatment time.

Methyluracil - instructions for use

Each dosage form the drug is used in compliance with dosages, duration of treatment, etc. Consider the rules for the use of tablets, ointments and suppositories of Methyluracil.

Methyluracil tablets

Tablets are drunk after or during a meal. Dosage depends on age:
  • Adults and adolescents from the age of 14 take Methyluracil 1 tablet (500 mg), 4 to 6 times a day.
  • Children from 3 to 8 years old take half a tablet (250 mg), 3 times a day.
  • Children from 8 to 14 years old take half or a whole tablet (250 mg or 500 mg), 3 times a day.
At the same time, adults can take a maximum of 3 g per day (6 pieces) of Methyluracil in tablets, children 3-8 years old - 750 mg (1.5 pieces), and children 8-14 years old - 1.5 g (3 pieces).

The duration of the course of therapy for organ damage digestive system(stomach and duodenum, peptic ulcer, enteritis, etc.) is 30-40 days. Therapy of diseases gastrointestinal tract is the longest. Therefore, in the treatment of other pathologies, Methyluracil tablets are used in shorter courses, the duration of which is determined by the rate of recovery and the normalization of a person's well-being.

Methyluracil ointment

The ointment is used to accelerate the healing of wounds and violations of the integrity skin various origins(injuries, stitches, etc.). The daily dosage of Methyluracil ointment is 5-10 grams, depending on the area to be treated and the frequency of dressing changes.

Methyluracil can be applied to any wound surface to accelerate the recovery of normal tissue structures. To do this, first, the seam, wound or burn is treated with antiseptic solutions - washed with 3% hydrogen peroxide, furatsilin, potassium permanganate, chlorhexidine, 70% alcohol, etc. When processing, all purulent and necrotic masses are thoroughly washed out of the wound. Intact skin around the wound or suture is also treated. After that, an ointment is applied directly to the wound surface, to the seam, to the cut, to the burn, which is covered with sterile gauze on top. If the wound is actively cleaned, there is a lot of pus, exudate or necrotic tissue in it, then such treatment with the application of a new bandage is carried out every 4 hours. If the wound is clean, then it is optimal to treat and apply a new dressing twice a day - in the morning and in the evening. The duration of the application of the ointment depends on the rate of restoration of the normal structure of the tissue. Usually postoperative suture under the influence of Methyluracil heals within 4-5 days.

Women use Methyluracil to accelerate the healing of the vaginal mucosa and perineal sutures after childbirth. Seams must be properly processed, following the following sequence of actions:
1. Wash with soap.
2. Wash the seams with an antiseptic solution (potassium permanganate, furatsilin, hydrogen peroxide, etc.).
3. Dry the skin gently with a soft, clean cloth.
4. Lie on the bed without wearing underwear, and dry the skin of the perineum in the air for 15 minutes.
5. Squeeze methyluracil ointment onto sterile gauze and apply it to the sutures.
6. Wear clean, natural lined underwear.
7. Depending on the number of lochia, change gauze with ointment after 2 to 6 hours.

Methyluracil ointment is also used to heal microcracks and ruptures of the vaginal mucosa after childbirth. Women inject the ointment into the vagina in two main ways. Before administering the drug, you should wash yourself vaginally and douche with a weak solution of potassium permanganate. After that, dry the perineum with a soft and clean cloth. Then insert the ointment into the vagina with a finger or a swab. If a woman can squeeze a little ointment onto her finger, insert it into the vagina and lubricate the mucosa in a circular motion, then this option can be used. Otherwise, apply about 5 cm of ointment to a cotton swab and insert it into the vagina. It is necessary to use Methyluracil in this way 2-3 times a day, until the mucous membrane heals and the woman feels normal (usually this interval is 4-7 days).

Many women cannot insert a tampon with ointment into the vagina - it is smeared, drains, etc. To facilitate the introduction, it is necessary to lie on your back, spread your legs bent at the knees, and raise the pelvis, remaining in this position for 2 to 3 minutes. After a while the muscles pelvic floor relax, and the tampon can be easily inserted, remaining in the described position.

Methyluracil suppositories

Candles (suppositories), according to the instructions, are intended for introduction into the rectum. However, doctors often prescribe Methyluracil in the form of suppositories in the vagina. But the manufacturer's instructions contain official information, taking into account the fact that the suppositories were not developed for use in gynecology.
